α-phe HSSE Unsafe Act Unsafe Condition

Unsafe Act Unsafe Condition (UAUC) is a safety programme where any individuals within company employee and contractor employees are empowered to take immediate corrective actions whenever possible, to overcome observations of unsafe act, condition or both and report those unsafe act or condition observed and action that had been taken.

Management shall actively encourage the reporting of unsafe act or condition since this programme had been known for its effectiveness in reducing hazards that may lead to incident, at any workplace.

These UAUC reporting can remain anonymous so that more people are encouraged to do the reporting at all levels. HSE committee and management should act upon these reports to build a safety culture within the company.

A record of all UAUC reports is kept in the α-phe UAUC Module. These records should include date, nature of report, recommendations and closeout.
